Continue with good hydration, handwashing and wear sunscreen when outdoors
Exercise and exertion:
Do not lift more than 10 pounds for 8 – 12 weeks post-transplant; slowly increase weight as tolerated each week.
Avoid core exercises (such as crunches) for a few months post-transplant.
Light cardio as tolerated after one month post-transplant.
At 6 weeks post-transplant: this is the end of quarantine. Your child can be fitted for kidney protector and we will coordinate your visit with a nutritionist in clinic.
Vaccines and medications:
At 6 weeks post-transplant: We may consider decreasing or discontinuing Nystatin at this visit. If a stent had been placed, it may be removed around this time.
At 3 months post-transplant: can receive non-live vaccines
At 6 months post-transplant: We may consider discontinuing Valcyte if your child was started on this antiviral medication
Patients 6+ months post-transplant
General:
Continue with good hydration, handwashing and wear sunscreen when outdoors
Follow-up visits at CHOP:
When patient reaches 14 years old: Teen Clinic every six months (this replaces the regular clinic visit)
Beginning one year post transplant: Hypertension Clinic every year or as recommended (this replaces the regular clinic visit)
Vaccines and medications:
Flu vaccine: every year
Gardasil: A three-dose schedule is recommended for patients age 9 to 26 who are immunocompromised.
Pneumovax 23: every five years x 2 doses (can be administered in clinic)
Clinic visit schedule
Transplant to 6 weeks post-transplant: two times per week
6 – 12 weeks post-transplant: one time per week
3 – 6 months post-transplant: every two weeks
6 – 12 months post-transplant: one time a month
1 – 2 years post-transplant: every four to six weeks
2 years post-transplant until transition to adult care: at least every three months
