All new employees are responsible for scheduling a pre-placement health evaluation. Call the Occupational Health Department (OHD) as soon as you receive your offer letter and orientation date. You can reach the OHD at (215) 590-1928 weekdays between 7:30 a.m. and 4 p.m. If you will be working at a satellite location (outside Philadelphia), and you received a packet from your recruiter, refer to the second set of instructions on this page. Failure to complete this requirement will result in a delay of your start date.

Any required vaccinations will be will be administered free of charge if you do not have adequate records.

Please attach the required proof of your immunizations (e.g. copies of baby book/school shot records, titers). The OHD nurse will contact you and your manager with directions for completing any deficiencies.
A urine drug screen must be completed at a Quest site if you are mailing your paperwork. Your packet contains the Urine Drug Test Form. You can access a list of sites at Quest's Web site. NOTE: YOU MAY NOT USE A THIRD PARTY SITE FOR YOUR DRUG TEST.
