Health Screenings
All new employees who complete both a health screening and an online health risk assessment will save on their medical insurance premuims. Use these forms to get health screening resuilts from your primary care provider or the Walgreens Take Care Clinic closest to you.

Walnut Street (for Wanamaker Building)
Wanamaker Fitness Center
If you are a CHOP employee based out of the Wanamaker Building, you are able to access the Wanamaker Building Fitness Center at no cost to you.
To gain access to the Wanamaker Fitness Center, you must complete the CHOP waiver form as well as the building's (Amerimar Management) waiver form. Once the forms are complete, place them in the bin on the reception desk (labeled "Fitness Center) of the Information Services office on the 7th floor of the Wanamaker Building.
The forms are be processed on a weekly basis and sent to the building management office to have access granted. Once your forms are processed (1-2 weeks), you will use your Wanamaker Building access card to enter the facility (the card you use to enter at the security desk in the lobby).
