Background

Brooke Heilman, LSW is a member of the Pediatric Advanced Care Team (PACT) at the Children’s Hospital of Philadelphia (CHOP) where she currently provides inpatient palliative care support.  She will also be the primary social worker for PACT’s upcoming pilot for community based palliative care (PPC @Home). She is passionate about supporting patients and families navigating complex medical needs in a way that is collaborative, trauma informed, and patient centered.

Brooke comes to the PACT team with prior experience supporting families within CHOP’s Division of Oncology and within the community as a school-based social worker. She holds a Bachelor of Social Work from Messiah University and her Master of Social Work from the University of Maryland. She has also completed extensive training in childhood trauma and trauma competent care through Lakeside Global Institute.
