Insurance and Payment Information
We participate with many insurance plans. Check your insurance provider directory or website to be certain our physicians and/or practice are participating providers. Please bring your insurance card to each visit.
It is recommended that you check with your health plan administrator to determine coverage for services. Our providers’ participation with your insurance carrier does not mean all services are paid in full by insurance. Any balance outstanding after the insurance payment is the patient’s responsibility. If our providers do not participate with your insurance, payment in full is due at the time of service. You will be provided with a receipt that can be used to submit a claim for reimbursement.
Fees
Many insurance plans call for a patient co-payment which is required to be paid at the time of service.
- We accept cash, check, Master Card, Visa, Discover and American Express.
- Prompt Pay Discount (20 percent discount for charges when payment is made at the time of service) applies to uninsured families (self pay) or services that are not covered by your insurance plan. This does not include deductible or co-pay obligations.
- If you feel you have special financial circumstances, please contact our practice manager to make individual arrangements.
