Insurance Information

It’s always prudent to check with your healthcare insurance provider to determine your coverage for services. CHOP Care Network/ Harborview/Smithville, participation with your insurance carrier does not always mean services are paid in full. Any balance after insurance payment is the patient’s responsibility.

Please be sure to bring your insurance card(s) and required co-payment (if any) to each appointment. It is also important that you contact your insurance company and alert them that CHOP Care Network/Harborview or one of our physicians specifically will be serving as your child’s primary care physician.


Many insurance plans call for a patient co-payment which is required at the time of service.

Medical insurance requires birth notification of your child within 31 days of delivery. The insurance company will not pay without proper notification. Contact your insurance company to advise them of your child’s birth and bring your insurance verification to the visit.
