Health Forms and Referrals

We will be happy to complete your child's health form. In most cases, your child must have been seen here for a complete check-up within the past 12 months for us to complete these forms. If this is not the case, an office visit for the check-up will be required.

Health forms are completed at the time of your child's physical exam. A fee will be charged if the form is completed at a time separate from the physical. Please allow 7 working days to have these forms completed.

Referrals to specialists take three to five working days to complete.
