A fundamental goal of the LEND program is to develop community partnerships that serve to enhance the health infrastructure of our community, city and region. The Community Education and Liaison component of the fellowship has two objectives:
Through the LEND Community Education and Liaison Program, fellows share their expertise in the service of improving resources for children with neurodevelopmental and related disorders and their families.
Under the supervision of the Coordinator of Community Education and Liaison, fellows select one option from a range of projects through which they will provide community services related to children with special needs and their families. In addition, fellows are required to develop and give at least two professional presentations for community-based agencies or parents' groups over the course of the training year. At least one of the community presentations will be given as part of the LEND educational series offered through the following CHOP initiatives: the Homeless Health Initiative, the Early Head Start Program, the Transition to Adulthood Program, REACH and others.
REACH (Rapport, Empowerment, Advocacy, through Connections and Health) is a program for teens and young adults ages 12-24 with special health care needs and their families, created by teens and young adults with special health care needs. REACH was created in May, 2003.
Mission
To prepare teens and young adults with special health care needs for a successful transition into adulthood by providing peer support, guidance and resources.
Vision
REACH will locate and/or help to develop comprehensive, collaborative adult healthcare and community services that encourage and support independence and well-being for young adults with special healthcare needs.
REACH offers mentorship and training to help youth acquire the skills needed to develop and maintain partnerships among families, youth and providers as they move through transition.
The REACH Program sponsors monthly events during the academic year. All events are on Saturdays, from 11 a.m. to 2:30 p.m. and include lunch. All events will be held at 3535 Market Street, Philadelphia, PA, 16th Floor.
REACH events are open to parents, teens, and young adults (ages 12 to 24) with chronic illnesses, physical disabilities and other special healthcare needs.
