Pediatric Anesthesiology Fellowship Objectives
Recognizing that no physician emerges from training with a comprehensive understanding of every conceivable aspect of pediatric anesthesiology, the objectives are stratified according to the level of proficiency that is expected at the end of the pediatric anesthesia fellowship:
Cognitive
Comprehensive mastery
- Preanesthetic assessment and preparation of the pediatric patient
- Pediatric and developmental anesthetic pharmacology
- Developmental anatomy and physiology pertinent to anesthesiology
- Interpretation of pediatric laboratory results
- Assessment and management of the normal pediatric airway
- Intraoperative temperature regulation and maintenance
- Perioperative fluid therapy
- Implications of common diseases and surgical interventions in infants and children
- Postoperative pain management assessment and therapy
- Assessment and management of common pediatric postanesthesia care unit problems
- Advanced life support for pediatric patients
Significant experience
- Congenital heart disease: assessment and perioperative management
- Assessment and management of the difficult pediatric airway
- Methods and goals of mechanical ventilatory support
- Management of massive fluid and/or blood loss
- Pharmacological support of circulation
- Considerations for anesthesia management during diagnostic/therapeutic procedures outside the operating room complex
- Anesthetic implications of major pediatric surgical interventions
- Neonatal emergencies
- Solid organ transplantation
- Craniotomy
- Craniofacial reconstruction
- Scoliosis repair
- Understanding of chronic pain conditions and management options
- Recognition and treatment of perioperative vital organ dysfunction
- Transport of critically ill pediatric patients
- Understanding of the psychological impact of serious medical conditions and surgery on pediatric patients and their families
Introductory familiarity
- Pediatric critical care medicine
- Anesthetic assessment and plan for uncommon conditions or procedures with uncertain implications (e.g., fetal surgery)
- Appropriate consultation for other specialists
Psychomotor
Comprehensive mastery
- Management of the normal pediatric airway with and without tracheal intubation
- Vascular access for fluid and pharmacological therapy
- Common peripheral nerve blocks
- Intraoperative placement of caudal and lumbar epidural nerve blocks with and without catheter
Significant experience
- Management techniques for the difficult pediatric airway
- Vascular access for invasive hemodynamic monitoring
- Nerve blocks for anesthetic management
Introductory familiarity
- Airway management for one-lung ventilation
- Intraoperative thoracic epidural nerve block with catheter placement
- Nerve blocks for chronic pain therapy
Affective
- Psychological response of the pediatric patient about to undergo anesthesia
- Psychological response of the adult(s) caring for the pediatric patient about to undergo anesthesia
- Relationships among physicians, paraprofessional staff, pediatric patients and others caring for them
