Registration
We ask that you arrive at the Children's Seashore House, on our Main Campus, twenty minutes prior to your appointment to allow enough time to register. During registration please have the following information ready:
- Your child's social security number
- Your child's medical insurance card
- A referral or prescription for services if your insurance provider requires it
- Please call our financial counselor at 800-664-7855 to see if we accept your medical insurance plan.
- If your demographic information has changes, please call 800-977-3734 prior to your visit.
