To make a new patient appointment, please submit the Request an Appointment form. This is the most efficient way to set up an appointment for your child. The first page asks for demographic and contact information. The second page gathers information about your child's needs. To complete the form:
The Department of Child and Adolescent Psychiatry and Behavioral Sciences has a central intake office for all programs (including the Center for Management of ADHD) and all locations. You should expect an email response from our department within a few business days. Due to the high volume of requests, there is often a wait of several months before your child's initial visit is scheduled. We know this can be a difficult time for you and your child, and we thank you for your patience.
For all other types of requests including follow-up appointments, prescription refills, medical records or billing inquiries, please call 215-590-7555 and select the appropriate prompt.
If your child is in immediate risk of harming him/herself or others, please call 911 or go to your nearest crisis center or emergency room.
The Department of Child and Adolescent Psychiatry and Behavioral Sciences has agreements with numerous insurance plans. Each of the agreements is different, and coverage varies depending on services provided. Visit "Does CHOP Take My Mental Health Insurance?" to learn more.
