Cardiology fellowship
To apply for a fellowship in Cardiology, download and print the PDF version of the application. Complete the appropriate fields and mail the application to the address listed at the end of the form.
Eligibility
Physicians who will have completed residency training in a program approved by the American Board of Pediatrics or who will otherwise be eligible for board certification may apply for the Fellowship in Pediatric cardiology.
Application requirements
The following must be received prior to consideration of your application:
- A completed application form
- Educational Commission for Foreign Medical Graduates (ECFMG) certificate number and visa information, if a foreign medical graduate.
- Three letters of recommendation from people familiar with your professional work.
- Medical school transcript
- Curriculum vitae
- Applicants are encouraged to submit USMLE scores. Successful applicants must have passed all three portions of the USMLE prior to beginning the fellowship.
All application materials must have been received by November 16, 2009. An interview may be scheduled after receipt of the above documentation. Our program will be participating in the National Resident Matching Program (NRMP)-sponsored pediatric cardiology match for fellowship beginning July 1, 2011. The timetable for the Web-based match is as follows:
- Match registration opens: January 20, 2010
- Rank order list system opens: April 14, 2010
- Quota Change Deadline: May 5, 2010
- ROL Closes/final deadline: May 19, 2010 (The system closes at 9 p.m. eastern time on this day)
- Match Day/Results: June 2, 2010 (Results available via the web at 12 noon eastern time)
