During the initial call to the Diagnostic and Complex Care Center, you will be asked a series of questions regarding your child’s condition: How long has your child had his condition? What are your child’s symptoms? Who is referring you to our Center, and has your child been seen by any other specialist? At that time a detailed intake will be completed. We will also need your child’s insurance information and the information of the insurance subscriber.
Prior to scheduling an appointment with our center, we ask that your child’s pediatrician write a brief but descriptive summary on why he/she is referring your child to the Diagnostic and Complex Care Center. It is also very helpful if you, the parents, are able to provide us with a detailed timeline of events that has occurred during your child’s condition. We also ask for a copy of your child’s medical records to be faxed or mailed to us. Once we receive this information, a physician will review the records. At that time you will be contacted by one of our staff members to schedule an appointment.
Once your child’s records have been reviewed, we generally schedule the first appointment within two to three weeks.
Yes, we do have a “wait list” for our patients to be added for a sooner appointment.
