The Healthy Weight Clinic is a clinic for children ages 4-17 years who meet the clinical definition of overweight or obesity and have at least one of six qualifying co-morbidities. The clinic also accepts children whose BMI is at or over the 99th percentile with or without weight-related comorbidity.
The purpose of the multidisciplinary clinic is to develop a complex care plan in partnership with the primary care provider, assess medical needs through a comprehensive assessment, coordinate care with other specialties and provide weight management through family-based lifestyle changes.
With proper documentation of relevant tests or lab work*, overweight or obese children may qualify for the Healthy Weight Clinic if they have one or more of the following co morbidities:
Children whose BMI is at or above the 99th percentile may be referred without a weight-related comorbidity.

At the present time, the Clinic is not be able to serve children with the following special needs:
Clinic referral forms are available online or by request at 267-426-2782. Referral to the Clinic needs to come from the patient's primary care provider.
