Making a referral to the Healthy Weight Clinic

The Healthy Weight Clinic is a clinic for children ages 2 - 18 who meet the clinical definition of overweight or obese.

The purpose of the multidisciplinary clinic is to develop a complex care plan in partnership with the primary care provider, assess medical needs through a comprehensive assessment, coordinate care with other specialties and provide weight management through family-based lifestyle approach.

How to refer to the Healthy Weight Clinic

Please instruct families to call 267-426-2782 to schedule an appointment. Consult request forms are no longer required.

Fax an order for “consult to Healthy Weight” to 267-426-6012 or give to the family to bring to the first visit.

Patient visits

Over the course of 2 visits, families will see a pediatrician that specializes in nutrition, a nurse practitioner, a nutritionist and a social worker. Generally patients will see the pediatrician and social worker at the first visit; and the nurse practitioner and dietitian at the second visit. After the second visit, the clinic team creates a follow-up that will ensure families are able to meet their health and weight management goals.
