Tell your primary care provider you’re interested in the clinic. Call 267-426-2782 to schedule an appointment.
If your insurance plan requires a referral, please bring it to the visit with you. We may need to reschedule your visit if we do not have a referral at the time of the visit.
We will work with your child's primary care provider to get test results from previous visits. If your child has had any testing by another physician, please try to get that information to us at least two weeks before your scheduled visit. This allows Healthy Weight Clinic providers to review the results before seeing your child.
Some insurance plans will not cover laboratory studies performed at Children's Hospital. Check with your insurance company or primary care provider before your visit.
About two weeks before your visit, you will receive a packet of information in the mail which will include a clinic information sheet, insurance referral information, and family questionnaires.
Your family will also receive a weight management information sheet.
Here are some reminders for your visit:
Over the course of 2 visits, you will see a pediatrician that specializes in nutrition, a nurse practitioner, a nutritionist and a social worker. Generally your child will see the pediatrician and social worker at the first visit; and the nurse practitioner and dietitian at the second visit. After the second first visit, we will create a follow-up plan for more visits that will help your child meet their health and weight management goals. We will work together to make sure your visit is as short as possible.
You can expect traffic, construction and parking delays. Allow enough travel time to get to your appointment on time. If you are late, we may need to reschedule your appointment. Please read our new arrival and late policy, effective September 1, 2010, prior to your appointment. On the day of your appointment, call us at 215-590-4983 if you are running late.
The Healthy Weight Clinic is located at 3550 Market Street on the first floor.
Even if the Clinic cannot see your child, there are still steps you can take to help a child who is overweight. There are also many people who can help.
