Children's Hospital Home Care is an important link in the continuum of care for many children with acute and chronic illnesses. We serve patients from infancy to adulthood and work closely with your child's primary care providers.

We coordinate home care services, equipment and therapies for CHOP patients and patients who are treated by other healthcare providers.
