Care Coordination
All patients receive care coordination as appropriate to the care, treatment and services provided. Patients are followed by one or several members of the Home Care interdisciplinary team throughout the continuum of care, from Hospital to home and through subsequent readmissions.
Our care coordination team includes:

- registered nurses
- respiratory therapists
- pharmacists
- dietitian
- social worker
- patient access coordinators
- clinical support staff
- clinical operations managers
- operations support staff
- the patient's physician
Many of these professionals have more than a decade of home care experience.
