Our Services
The Home Care Department of The Children's Hospital of Philadelphia plays an important role in your child's care by coordinating treatment between the Hospital and home, and providing a full range of services in your home.
Care coordination
Your child's care will be coordinated to ensure we are providing the care, treatment and services that meet his or her individual needs. Our patients are followed by one or several members of the Home Care interdisciplinary team throughout the continuum of care — from Hospital to home and through subsequent readmissions.
Our care coordination team includes nursing patient care coordinators, nurses, respiratory therapists, pharmacists, a dietitian, a social worker, patient access coordinators, clinical support staff, operation support staff, operations and clinical managers, and your child's physician.
