Our Home Care Program
Children's Hospital Home Care, a department of The Children's Hospital of Philadelphia Care Network, is dedicated to providing family-centered care in the home as an alternative to hospitalization and as an extension of care delivered in the outpatient setting.
Our mission
We provide safe, innovative, fiscally responsible home care services to patients from infancy to adulthood that improves their health outcomes and positively impacts their quality of life. The goal of Children's Hospital Home Care is to provide high quality, comprehensive home care services to patients and families.

Our interdisciplinary team provides an important link in the continuum of care between the Hospital and the community.
