The first step in a child’s journey to The Children's Hospital of Philadelphia is to provide our physicians with as much information about the patient’s condition as possible. We ask that families fill out and return our Patient Intake Form (English, Arabic) along with the child’s medical records, including specialists’ reports, history and physical, lab, radiology and pathology reports. All information needs to be in English. - Help
At any point during this process, our International Patient Services staff is here to assist with completing the intake form, obtaining records or answering any questions. Families, physicians or embassy representatives can reach us directly by telephone, email or fax. Language services are also available for these communications. - A plan
Once the child’s information is complete, our physicians will carefully review it and determine if the patient can benefit from consultation and treatment at The Children’s Hospital of Philadelphia. If so, we will provide a recommendation about the child’s specific needs and proposed treatment plan.
If the family wishes to arrange for care at CHOP, we will discuss financial arrangements based on the patient’s insurance or the family’s preferred method of payment. We will provide documentation confirming a treatment plan to embassies to assist with visa applications and begin to schedule the necessary appointments for the child at CHOP.
Financial matters
We want families to have as much information as possible when making decisions about their children’s care. Our experienced staff can help families understand what charges are anticipated for our services, and work with them to facilitate payment in advance of consultation, whether through international insurance or self payment.
