Before You Leave Home

1. Collect all of your child’s medical records, in English.

In order for us to provide the best care for your child, it is critical that you bring complete, up-to-date medical records for your child. This should include but is not limited to:

  • a detailed medical history including a summary letter from the physician supervising your child’s care
  • results of any diagnostic tests, as well as actual images and pathology slides and reports a list of your child’s medications including names doses and preparations
  • contact information for all primary care and specialty physicians
  • a family medical history

When the medical team has access to all this information ahead of time, they can often schedule multiple visits for your child in one day, and get more of your initial appointments done in less time.

2. Verify your visa requirements.

Visa requirements vary by country. Unless you hold a passport from a country participating in the Visa Waiver Program, you must obtain a visa to be admitted to the United States. For country-specific information, please contact your embassy or visit the U.S. Department of State website. Online Visa applications are also available.
