    All tissue removed during surgery or surgical procedures.

    Room temperature in 10% Formalin.

    Stability

    10% formalin is stable at room temperature or at 20 c.

    Surgical Pathology Request Form without required information.

Internal -Must be submitted on a fully completed Surgical Pathology Request Form. Include all required information. Take to Pathology Monday - Friday 0730-1700. Weekends, Holidays and after 1700 take to Central Lab services. Include all required information. Send to Anatomic Pathology, Monday - Friday
