Remarks

Coordinate all requests with the Anatomic Pathology department Monday - Friday 0800-1700. Forms required: Permission for Autopsy, Copy of completed Death Certificate, Copy of Mother and Neonate charts,Summary of Clinical History.Include all required information and signatures. Transportation of body must be arranged by the Pathology department of the referring institution. Must include contact information of the person responsible for disposition of the deceased. CHOP will not take responsibility for disposition of the body.
