Monday-Friday transport immediately to Anatomic Pathology Room 5NW65. Weekend, holiday and after 1700 contact pathologist on-call and take to Central Lab Services.

Tissue placed in fixative, incomplete requisition form.
Specimen Handling
Place fresh specimen in required container without any fixatives and/or transport media. Contact Pathology (x4-1728) and immediately transport to Anatomic Pathology (Room 5NW65). Must have completed requisition form with all required data.

Remarks
Must be submitted on a fully completed Surgical Pathology Request Form. Include all required information. Take all fresh specimens mentioned above directly to Pathology Monday - Friday from 0800-1700. Weekends, Holidays and after 1700 contact the Pathologist on-call, via the hospital intranet on-call schedule, then take the specimen to Central lab Services.
