Medical questions or concerns
The Pediatric Feeding and Swallowing Center is located in the Children's Seashore House at the Hospital's main campus. We also see patients at our Pediatric and Adolescent Specialty Care Centers in King of Prussia, PA and Voorhees, NJ.
Children may be referred to the Center by a pediatrician, family physician or therapist. Many families also seek our services directly when feeding issues are of concern. If you'd like your child to be evaluated in the Pediatric Feeding and Swallowing Center, please call 215-590-7500 to complete an intake questionnaire over the phone.
Please allow approximately two weeks for your benefits to be verified and for our office to call you to schedule an appointment. If you do not hear from us by that time, call us at 215-590-7491.
To better understand and help with your child's feeding problems, we would appreciate your responses to the questions in the Feeding History Questionnaire. Please take the time to complete this questionnaire and fax it back to us at 215-590-9338 prior to your initial appointment. Please also bring a copy with you to your appointment.
Please complete and bring the Follow up Appointment Questionnaire with you to follow-up appointments.
The structure of our team is based on the model that feeding difficulties are caused by several interrelated issues.
