How to Apply

You may apply by downloading a PDF version of the application. Print the application, complete the appropriate fields and then mail to the address listed at the end of the application form.

Instructions for application

The fellowship program seeks physicians who have or will have completed training in Neurology or Child Neurology in an accredited residency program through the American Board of Psychiatry and Neurology.

The following should be sent to Dennis J. Dlugos, MD:

  1. Completed application
  2. Three letters of reference
  3. Curriculum Vitae
  4. Cover letter
  5. Parts 1, 2 and 3 of the USMLE taken within the seven year time frame as required by the Commonwealth of Pennsylvania or equivalent scores
  6. If a graduate of a medical school outside the United States, Canada or Puerto Rico, valid ECFMG certificate or one that does not expire prior to the start of the fellowship
  7. If not a citizen of the United States, copy of permanent resident card or copy of current, appropriate visa
