Transportation assistance to medical appointments 

Pennsylvania

Medicaid/PA Medical Assistance contracts with van/cab transportation companies to provide rides to and from medical appointments. They can provide door-to-door service if there is a medical reason that the child is not able to use public transportation. Medicaid is billed for the ride.

If your child is not able to ride on regular public transportation, your physician will need to complete a form to documentation your child's medical issues. You should contact the transportation company in the county where you live because each county program has different documentation needs.

Medicaid will reimburse families for money spent on public transportation to and from medical appointments under certain circumstances. Please contact the transportation company in your area for further information.

New Jersey

The state of New Jersey contracts with LogistiCare Medical Transportation to provide transportation assistance to and from medical appointments. The company provides upper-mode non-emergent Mobility Assistance Vehicles (MAVs), Ambulance service and lower-mode, livery service for Medicaid recipients in all counties of New Jersey.

Delaware

LogistiCare Solutions manages non-emergency medical transportation for all Medicaid and Chronic Renal Disease Program members in the state of Delaware. All trip reservations must be scheduled at least 48 hours in advance. Urgent trips may be scheduled with less notice if the treating facility authorizes the need.
