If your child is between 2 and 16 years of age, has a diagnosis of asthma, and lives in one of the zip codes we serve, we may be able to offer home visits to your family. CAPP's Home Visitor Program helps address the home environments of asthmatic children. In this program, trained lay-educators, or "home visitors," visit families of asthma patients in inner-city Philadelphia.
The Community Asthma Prevention Program currently serves the West, Southwest, South and North Philadelphia areas of the city. In order to receive home visits, your child must have persistent asthma and be on controller medicine.
