The care of children with medical complexity largely resides within the medical home in primary care. This course will help primary care providers better recognize and manage the chronic health problems and functional limitations of their patients in a team-based approach. Although children with medical complexity represent a small portion of our patient population, they account for a large number of pediatric visits, resources and specialty services. This course seeks to help primary care providers improve the quality of care for their patients with medical complexity, while reducing healthcare costs and supporting families.
