Have Your Chosen Your New Diabetes Care Team?

You may have already heard that the Diabetes Center at CHOP is enhancing its team approach to care. Each family will pick a team — based on the day, location or providers you prefer.

If you've already picked your team, great! If not, keep reading.

Your appointments will be every three months with the same team of care providers. Our full-service, team-based approach will help you manage your diabetes and achieve your personal best.

Each team will have: an endocrinologist, a nurse practitioner, a certified diabetes educator (CDE), a social worker, a dietitian and our psychologist.
