Patient Experience Survey Coming to Clinic

As part of our ongoing goal to provide the best care possible and to collect required data, all patients between the ages of 12 and 22 will be asked to complete a patient experience survey when you come to clinic for your annual comprehensive care visit.

Each child in a family needs to complete a survey. Parents or guardians of children ages 12 to 17 will be asked to complete the survey with their children. Patients 18 to 22 years old should complete the survey on their own.

This quality improvement initiative has been implemented to help us collect needed data on bleeding disorders patients for our Healthy People 2020 program grant from MCHB, a federal agency. You can complete the survey online using Survey Monkey or on paper, if you prefer. You will be asked to complete this survey every year from now on.
