Sally Poliwoda, BSN, RN, was recognized with the Mayor’s Philly Hero Award for her impact on our community. Poliwoda is a nurse at Children’s Hospital of Philadelphia (CHOP) and the leader of the Community Nurse Advocacy Fellowship, a program that places skills-based CHOP nurses in homeless shelters, schools, and other local non-profits to serve the community.
Since 1988, Poliwoda has volunteered for the Homeless Health Initiative, a program that consists of CHOP doctors, nurses and social workers who offer medical checkups at area shelters.
Mayor's Honorees were nominated by family, friends and peers for their dedication to being a force for positive change in Philadelphia. The award was presented on April 3, 2019, during the 7th Annual Mayor’s Day of Service Award Ceremony
