Description
The goal of this project is to design and test a way for families, who are caring for children living with serious illness, to regularly share information with their palliative care team about their care experiences and care needs while at home. Our hope is for care teams to use this information to better meet children's and families' needs while they are at home.

What to expect
Participants will take an online questionnaire every 3 months for up to one year. The questionnaires should take less than 20 minutes to complete each time.
