On-the-Go Info Sheets: Infectious Diseases & Vaccines

The “On-the-Go sheets,” developed by the Vaccine Education Center, can be viewed online, printed or photocopied. Healthcare providers who choose to can order On-the-Go sheets for their practice.
The Vaccine Education Center is pleased to offer these “On-the-go Info Sheets,” so families can quickly see a summary of information about vaccine-preventable diseases and the vaccines that prevent them. These sheets are available for online viewing or can be photocopied for distribution. This information is also available in both English and Spanish webpages on the Parents PACK website.
We also offer a QR code sheet in English and Spanish that can be displayed or shared for families to access our various "On-the-Go" info sheets.
