The Cardiac Center Nurse Navigators, Lauren Boldizar RN and Liz Hunter RN, both experienced Cardiac Intensive Care nurses, are a team exclusively dedicated to 
helping all referring physicians, and new patients and their families, connect with the Cardiac Center at CHOP. This support service expedites and facilitates communication for patient consultations, referrals, second opinions and transfers.
Liz and Lauren are a team dedicated to patients coming to CHOP from around the country. They are here to:
- Coordinate care for referring physicians, and for patient families, to simplify access, improve communication and enhance the patient and family experience at CHOP.
- Coordinate a plan of care for new patients and be a primary point of contact with referring physicians.
- Maintain a relationship with the patient family throughout the course of their care, from initial contact to the end of their hospitalization, providing the patient and referring physician with updates and pertinent medical information.
- Meet with local and regional referring physicians, and their staff, to help navigate the Cardiac Center and address any concerns.
