The Diagnostic and Complex Care Center (DCCC) at The Children's Hospital of Philadelphia is designed to meet the needs of patients with complex or difficult-to-diagnose illnesses. Our physicians and staff work closely with referring physicians in managing the your child's overall medical care.
For children with difficult-to-diagnose illnesses, our team provides consultation with a referral back to the referring physician or to a specialist if necessary. Our team coordinates consultations with subspecialties, evaluates subspecialists’ findings and facilitates communication of any recommendations to you and the referring physician.
During your visit, we will discuss your child’s medical condition in depth and allow for adequate time to answer your questions. Our physicians may order testing as well as determine which other specialties should be involved in your child's care, and assist you in arranging these consultations. At times, your child may require hospital admission for inpatient care, the Diagnostic Center team will help you coordinate such admissions.
For children with complex chronic medical conditions, our physicians work closely with outpatient and inpatient physicians and specialists to coordinate medical care and ease the transition from the Hospital to the outpatient setting. In addition, they provide support and consultation to the primary care providers regarding your child's unique care needs, support the establishment of a medical home and are dedicated to improving the flow of information.
In the case of a child with complex medical needs, effective communication is essential to good patient care. After each visit, a detailed letter is generated and provided to you, the primary care physician and consulting specialists. This letter serves as a concise medical summary of your child’s medical condition and current management strategies, and includes a list of all specialists involved in your child’s care.
Our physicians are supported by a nurse coordinator who provides an easily accessible resource for you, your child and referring physicians. In addition, referrals for social work services are available through our center.
