Transition to Adulthood

For children with special healthcare needs, who have received services from pediatric providers for their entire lives, the process of taking control of their own healthcare needs as they get older --and eventually switching to an adult healthcare provider-- can be challenging. But with the right information and support, it can be a time of positive growth for both patients and their families.
