Appointment schedules are based on your child's needs but typically occur at 6 months of age, 1 year, 2 years, 4 1/2 to 5 years, 6 years and then every two years thereafter.
Appointments in the PHP are designed to allow for visits with multiple specialists in one location. In most cases, any necessary testing can also be completed on the day of your appointment.
If your child was treated in the N/IICU at CHOP, our scheduler will contact you to set up your first appointment. If your child was not a CHOP patient, or if you have questions about our PHP program, please call us at 215-590-2733.
The Pulmonary Hypoplasia Program is located within the General Surgery Clinic on the 5th floor of the Richard D. Wood Ambulatory Care Center on CHOP's Main Campus.
When you come for a PHP visit, you will see General Surgery, Pulmonary Medicine, and Nutrition out of this clinic. If you are scheduled for Neonatal follow-up, you will also spend some time on the 6th floor of the Wood Center. Depending on the other consultations scheduled during your appointment, you might also visit Cardiology on the 6th floor of the Main Hospital, or Audiology on the 1st floor of the Seashore House in 1 East. Each specialty is located in close proximity and the PHP team will be there to help you get to where you need to be.
Proceed south on 34th Street, which becomes Civic Center Boulevard at the bend, to the third traffic light past Children's Hospital. Make a right at the light and proceed straight 100 ft. A ramp entrance to the underground garage is on your right, underneath the Wood Center. The garage is open 7 a.m. to midnight and accommodates vehicles up to 6' 7'' in height. Elevators and stairs connect the garage to the Wood Center.
