Young Adults The CHOP-Penn Transition Program includes adult and pediatric gastroenterologists, hepatologists, a full-time nurse practitioner and other healthcare providers from CHOP and Penn Medicine, working together to ensure a smooth transition to adult care.
Our team will work closely with you and your child to prepare you for the transition years in advance, and then provide comprehensive support during the process.
Our program offers services to patients transitioning from CHOP to Penn Medicine. We also assist students from colleges in the Philadelphia region and other young adults new to the area who are seeking care at Penn Medicine.

CHOP-Penn Transition Program features and benefits
The Transition Program nurse practitioner will:
- Meet periodically with you to begin preparing for transition.
- Provide information about being a young adult with a chronic illness, so you can make the safest choices. Topics include: working with college advisors so illness-related needs are met; sex, drugs and alcohol; health insurance; traveling; and much more.
- Talk to the pediatric specialist about your particular needs during transition.
- Help you learn about health insurance and prepare the necessary paperwork.
- Review the list of adult doctors at Penn Medicine and help you decide who might be the best fit.
- Serve as a liaison between the pediatric and adult specialist.
- Go along on the first appointment with the adult specialist.
