Your Child's Appointment with the Sinus Clinic

Prior to your child’s visit you will receive an electronic questionnaire through MyCHOP, our hospital’s online health connection. The questionnaire will take 10 to 20 minutes to complete and provides us with information that will make your child’s visit more efficient and effective. You do not need to complete it all in one sitting. If your child has seen other healthcare providers or had any imaging done to evaluate her or his problem, such as a CT scan, it is important to bring these records with you to the office visit.
