You are invited to participate in the Children’s Hospital of Philadelphia (CHOP) CATCH Program. Through this program, you will receive digital communication and follow-up related to your child’s care.
The digital communication is provided through a platform called Conversa, which is not owned or operated by CHOP. CHOP is not responsible for any defects in the Technology, interruptions in service related to use of the Technology, or improper disclosures of Protected Health Information (PHI) by Conversa or as a result of the use of the Technology.
The data you provide through the platform will be stored by CHOP in the Patient’s medical record and treated by CHOP like any other protected health information (“PHI”) under HIPAA. CHOP may use or disclose any PHI obtained through the Technology without my authorization for treatment (including with non-CHOP providers), payment, and internal operations.
