Uploading Data for Your Video Visit

Data upload instructions

It is very important that our team can review your blood sugar data during your visit with the Diabetes Center at Children's Hospital of Philadelphia (CHOP). For the best video visit possible, we recommend uploading your data before your visit. Below you will find instructions for different devices.

Continuous Glucose Monitors (CGM) and Meters

Dexcom

  • Customer assistance: 1-877-339-2664
  • Check out this video on connecting with Dexcom
  • How to upload your CGM to Dexcom Clarity:
    • Go to the Dexcom site
    • Click on Dexcom Clarity Home
    • Follow instructions to create patient account if you have not already done so
    • To upload choose either option:
    • View Data from Receiver:
      • Attach receiver to USB cord (also acts as your charging cord) and plug into your computer
    • Share Data from phone: Hit "Accept Invitation" in phone app & enter code that was provided by CHOP
      • Once the account is linked to CHOP Clarity account, data will be automatically uploaded
  • Click View Reports
  • Notify your diabetes team when you have uploaded your receiver (if using a receiver)

Libre sensors and meters

  • Customer assistance: 1-855-786-4263
  • How to upload your Libre Reader to LibreView:
    • Go to the LibreView site
    • Click “Sign Up”
    • Follow the 5 step registration process
    • Review data as a family
    • Notify your CHOP provider that you have uploaded and share your username and password OR send the report to us

Blood glucometers

  • For customer assistance email support@glooko.com
  • How to upload your meter to Glooko:
    • From a mobile device, install the “Glooko” app or navigate to the Glooko site here from any device
    • If you are using the app, sign up and follow the prompts
    • If you are using the website, sign in and follow the prompts at the bottom of the screen to sign up
    • Once signed in you will enter a ProConnect Code to share your data with CHOP
    • Click “Settings”, click “Account”, click “ProConnect, read disclaimer, click “Continue”
    • Enter “chopdiab” as the code and click “Submit”
    • Once the account is set up, you can use the same login in the Glooko app or go to the Glooko site and upload device from either the mobile app or website
    • Notify CHOP provider that you uploaded your device

Smart Pens

InPen

  • Customer assistance: 1-877-339-2664
  • Click here for a video on how to send InPen reports
  • How to pair your InPen with Dexcom Clarity:
    • In the InPen App go to "Settings" and select "Connected Services," then select the "+" in the upper right corner. Enter the Dexcom username and password
    • Check on Clarity to see if your InPen information is there

Pumps

Tslim (Tandem)

  • Customer assistance: 1-877-801-6901
  • How to upload your pump to T-Connect:
    • Go to the Tandem site here
    • Click on “Get Started”
    • Upload the TConnect Uploader for Windows or MAC
      • Make sure you download the right program for your specific operating system
  • Follow the three step process for getting Uploader onto your computer
  • Run the software and click “Upload Now”
  • After the upload is complete, view your reports under the “Reports” tab
  • Share Username & Password with CHOP Provider before your visit

Omnipod

  • How to upload your pump to Glooko:
    • First, check out this video for a quick tutorial
    • From a mobile device, install the “Glooko” app or navigate to the Glooko site here from any device
    • If you are using the app, sign up and follow the prompts
    • If you are using the website, sign in and follow the prompts at the bottom of the screen to sign up
    • Once signed in you will enter a ProConnect Code to share your data with CHOP
      • Click “Settings”, click “Account”, click “ProConnect, read disclaimer, click “Continue”
      • Enter “chopdiab” as the code and click “Submit”
  • Once the account is set up, you can use the same login in the Glooko app or go to the Glooko site and upload device from either the mobile app or website
  • Notify CHOP provider that you uploaded your device

Note: If you need a cord for your Omnipod PDM, please click this link to order. The cord is free of charge, but you do have to pay for shipping - it should be less than $5.

Medtronic

  • Customer assistance: 1-800-646-4633
  • How to upload your pump to CareLink:
    • Go to the CareLink site
    • Click “Sign Up Now”
    • Complete the registration process and log in
    • To upload data from your pump, click the “Upload” tab and follow on-screen instructions
    • Plug the Contour Next Link meter (USB) into the computer. Place the pump nearby
    • After upload is complete, view the following reports: Assessment and Progress, Weekly Overview, Sensor and Meter Overview, Logbook, and Device Settings
    • Notify CHOP Provider of your Username & Password
