When should I refer?

We invite you to contact us at any point in your patient evaluation process to discuss options or to seek advice on patient management. We serve as a resource for physicians, providing information to gauge when a child can be diagnosed and managed at their local institution, when CHOP specialists could provide guidance to the local care team, and when the patient’s condition requires a visit to CHOP.

We frequently consult with physicians both in our region and across the country, in many cases developing care plans that can be delivered completely at the patient’s home hospital.

How do I refer?

Please email cured@email.chop.edu the following to begin the referral process:

  • Insurance information
  • Medical summary/test results
  • HIPAA form
  • Name and contact information (including email address) for the case manager and social worker involved with the patient

Please note: Out-of-state referrals may require prior authorization and/or a negotiation of rates with patient's insurance company. The time frame can range from a few days to weeks.
