Sports Medicine Professional Referral Program

Professionals in the Philadelphia region who are members of the Sports Medicine Professional Referral Program at Children's Hospital of Philadelphia (CHOP) can submit patient information here for Sports Medicine appointments. You can expect a response within 24-48 hours, and likely sooner than that.

We encourage referrals from our partners in the community, including athletic trainers, physical therapists, advanced practice professionals, nurses, physicians, etc.

CHOP also offers pediatric urgent care at locations throughout the Philadelphia region. Our Urgent Care Centers are open until 10 p.m. and provide X-rays that are read by a pediatric radiologist while the patient and family wait.
