Transfer Guidance

Criteria for Evaluation at CHOP PHL and KOPH
  • Age < 18 yrs
  • Possible exceptions determined by individual case
    • Age ≥ 18 yrs with significant neurodevelopmental delay that are best cared for by a pediatric provider
    • Review with PEM APOD
    • SW, SART coordinators, SCAN are available to assist with decision as needed
CHOP PHL ED Transfer
to HUP
  • Age ≥ 18 yrs:
    • Complete MSE, stabilization as indicated
    • Possible exceptions determined by individual case
    • Transfer to HUP ED
    • Interfacility Transfer
CHOP KOPH ED Considerations
for Transfer
  • Age ≥ 18 yrs:
    • Possible exceptions to be determined by individual case
    • Exceptions as listed above
  • Provide Referral/Transfer options to one of the sites below. Contact referral site before transfer.
  • Interfacility Transfer
    • Responsible adult/caregiver present and has private vehicle:
      • Provide address/ phone number to referral ED
      • If not, offer transportation assistance (e.g., Uber or Lyft)
  • If requiring further medical needs and EMS transfer after stabilization, contact referral site transfer center.
Considerations for Transfer from KOPH to PHL Campus Requiring subspecialty care or additional resources not available at KOPH ED
