Acute Bacterial Rhinosinusitis Clinical Pathway – Primary Care, Emergency Department and Inpatient

Cohort

  • Children with suspected bacterial acute sinusitis
  • Not intended for symptoms caused by acute viral respiratory tract infections
    • Children < 4 yrs old are more likely to have a viral illness than ABRS

Diagnostic Criteria

Persistent Symptoms lasting more than 10 days without improvement
Worsening Symptoms worsen more than 5-6 days into viral URI;
“double sickening”
Severe Temperature ≥ 39°C and purulent nasal drainage for ≥ 3 days

Exclusions

  • Certain underlying conditions
    • Cystic Fibrosis
    • Primary Ciliary Dyskinesia (PCD)
    • Immunocompromised
  • Complicated sinusitis: concern for intracranial extension or
    orbital complications
