Functional Mobility Baseline

Use the medical history and parents’ interviews to understand what the patient’s pre-illness baseline functioning was. This is a screening tool for a typically developing child.

4 Months Turn head to rattle, follow past midline
Grab objects, roll from prone to supine
6 Months Turn head to voice, babble

9 Months Respond consistently to name
Nonspecific “mama”/”dada”
Plays “peek-a-boo”

4 Years Brush teeth, dress and undress with little help
Completely intelligible to strangers, play a simple card or board game
Hold pencil with fingers/thumb instead of fist, copy square
Use medium-sized buttons, go up-stairs without help
5 Years Follow simple directions, count to ten, know at least 4 colors
Print some letters/numbers, copy triangle
Stand on one foot for several seconds
