Asthma Clinical Pathway — Inpatient

Discharge Criteria

Respiratory Assessment
  • Mild
  • No supplemental oxygen required
Hydration
  • Adequately hydrated
  • Tolerating PO
Medication(s)
  • Prescribe medication(s) to CHOP Retail Pharmacy (p: 267-425-8888) or preferred pharmacy
  • Prescribe controller therapy at discharge, as clinically indicated based on review of Assessment of Asthma Control
  • Continue Yellow Zone rescue therapy until respiratory status returns to baseline
  • Consider flu shot during influenza season
  • Assess allergen triggers (e.g., pollen) and consider therapy at discharge, as clinically indicated
Education
  • Asthma education in class or at bedside completed and documented
  • Encourage parent/caregiver to pick up prescriptions prior to discharge teaching
  • Provide Asthma Action Plan to patient/caregiver
  • Educate patient/caregiver about Asthma Action Plan, using discharge medications during education
  • Consider Community Asthma Prevention Program (CAPP)
