All Patients are Consulted by Unit Social Worker
  • Social work assessment
  • Custody information
  • Liaison for family to medical team
  • Patient/family strengths and/or stressors
  • Coordinate with school and active BH and child welfare supports
  • Complete social work assessment tool in Epic
  • Safety plan, when appropriate
  • Coordinate patient/family meetings
  • Psych Cnslt via On-Call Finder
  • Varies by unit
  • Check with charge nurse or unit clerk
BHIP

Clinical Social Worker

Psychologist
  • Psychotherapeutic intervention for patient/family
  • Evaluation
    • Eating disorders
    • Mood disorders
  • Liaison with medical team for complex care cases
  • BHIP Coordinator:
    • Receives consult order
    • Assigns consult to appropriate BHIP provider
  • BHIP Providers:
    • Contacts medical team for further clinical information
    • Completes the consult
    • Makes preliminary level of care/treatment recommendations
    • Relays consult findings to the medical team
  • Place BHIP consult order in Epic
  • Psych Cnslt via On-Call Finder
Psychiatrist
  • Liaison with medical team for complex care cases
  • Guidance to medical team on complex capacity issues
  • Agitation plan, as needed
  • Medication management
  • Safety assessments
  • Consider higher level of care
  • Completion of 201/302
