Behavioral Health Issues Clinical Pathway — Emergency Department

Behavioral Health Screen (BHS) for ≥ 12 Years

The Behavioral Health Screen (BHS) screens for depression, suicidality, trauma, and substance abuse.

Eligibility Age ≥ 12 years of age with any chief complaint are eligible
RN Action
  • Bedside RN or ED Tech administers to eligible children
  • ED computers, iPads, or the child’s phone
FLOC Action
  • Reviews results, completes BHS documentation in Epic
  • Current suicidal ideation or severe depression on the BHS
    • Review with child, contact SW for continued concern
  • PTSD, Substance Abuse and other results
    • Review with child, contact SW as needed
  • Document BHS results and actions in provider note
Follow-up
  • Notify provider if BH follow-up is recommended
  • CHOP PCP
    • Send Epic in-basket message to the pool for the practice and cc: the PCP
  • Non-CHOP PCP
    • Call the PCP’s office and document this action in your note
